This application is to request a review of your test results or dispute any other condition of your testing that you think altered the outcome of your test. You must submit a detailed explanation with this Test Review Request. We recommend calling D&SDT-HEADMASTER staff for a test results consultation before submitting this form with your test review fee. NOTE: Please refer to the Candidate Handbook ‘Test Review Requests’ section.

A nursing student, or a nurse who is no longer actively licensed, may apply to request an equivalency waiver to opt out of the 75-hour nurse aide training program (NATP) requirement.

A CNA who does not have the CE or employment hours must retake the CNA knowledge exam and skills evaluation to be eligible to reinstate to an active status.
